We accept pediatric referrals from:

Parents, Pediatricians, Child Development Agency, and Other Agencies Serving Children.

We accept adult referrals from:

Primary Care Physicians, ENTs, Neurologists, Oncologists, PT/OT, self referrals, or other agencies serving adult populations.

We will need the following:

• Prescription Order/Referral – you may get this yourself or we can assist you with this request.
• Insurance Card – if you or your child have both private insurance and Medicare/Medicaid, please provide a copy of BOTH cards.
• ID – please bring a current, valid photo ID. 
• List of Current Medications and Allergies
• Payment Method
